Community Participation and Social Engagement

At Care Collective we understand that engaging and interacting with others is a crucial and joyous part of life. As one of NDIS most popular supports we will help implement your plan to ensure that you get out into the community, try new and exciting activities, meet other people and improve your overall well being and health. We strive to ensure we are well connected to service providers and various community activity organisations to find suitable interests and hobbies for each one of our clients. We are able to offer variety, safety and enjoyment by tailoring or searching to each individual client’s preferences. We have connections to individual programs or group activities. This is inclusive of clubs, hobbies, health improvement, learning new skills or volunteering for passion projects. It is our pleasure and role to support you through these activities and ensure your complete comfort, safety and engagement.

Development of Life Skills

Care Collective strives to enable participants and build their skill set to be able to confidently engage in activities of their choosing. This allows them to gain independence, confidence and control. We work collaboratively to gain an in-depth understanding of your goals, needs and support them to achieve this. This is included but not limited to the below:

Meal Planning and Preperation

We are able to help you with the preparation of healthy meal plans that meet all of your nutritional needs. Assistance can be provided through the entire stage of the process or just those key moments in which you may need support. We have trained staff that will ensure high safety and hygiene standards. We aim to build as much independence as the participant desires.

Travel Training in Road Safety and Public Transport

We aim to meet all of our participants’ needs both inside and outside the home. We are happy to plan clients’ journeys when accessing the community and are able to offer assistance through public means of transport or provide private transportation based on individual preferences. We further have access to great safety information and are able to relay this to clients in effective ways. We understand how important it is that clients are able to access the community safely.

Support to Establish Meaningful Relationships

Relationships in the form of friendships and loved ones are a crucial part of everyone’s lives. We understand that sometimes it can be difficult to start or maintain these relationships and therefore are able to provide guidance and assistance through forms of community groups, collaborative classes and team building exercises. We aim to leave our clients with wide social networks and friendships that share common interests.

Assistance with Daily Personal Activities

Care Collective are able to assist with or provide supervision for any daily personal activities that a client may need support for. The level of support will be determined by your NDIS plan and is available for clients within support accommodation or those clients who are living in their home. We pride ourselves on flexibility, adaptability and respect for your choices. Our team will be there to support you to live the way that you chose with as much independence as possible. We are able to support personal and oral hygiene, bathing, dressing and grooming, toileting bladder and bowel management, eating and drinking, use of active technology and aids/appliances, mobility and transferring and attending appointments. This is inclusive of social, educational, recreational and medical or theory appointments.

Household Tasks

In accordance with your NDIS plan Care Collective offers a wide array of services to maintain and help support all aspects of household activities and cleaning. Qualified support workers are able to aid participants to ensure your home is clean, comfortable and safe. We strive to create an environment that encompasses all of your choices, expectations and standards.

Our support workers are friendly and trained to complete cleaning of all surfaces (mopping, vacuuming, dusting, window cleaning), organisation of all household items (linen, groceries, kitchenware, clothing), kitchen duties (groceries, meals and cooking, dishwashing).

Support Coordination

Care Collective Sydney offers support coordination to allow clients to utilise their entire NDIS budget in the most beneficial and optimal way. Through our wide array of resources and networks we will alleviate the stress and pressure of having to source and identify trustworthy and skilled therapists, intervention programs and all assistive technology devices. We will support you in navigating the complexities of the NDIS system and allow you to focus on achieving your goals by arranging and organising all of your support.

Support coordination is further able to assist your using your plan building meetings with NDIS partners and representatives to ensure that you are receiving all the support you are entitled to and receive as much value as possible. We act as firm advocates and through experience working with the NDIS are able to organise resources and supporting evidence to back up your funding requests.
